Michigan 4-H Foundation 4-H Participation Scholarship Grant Application Form

Each county should only apply once. Staff should complete one collective form for the county 4-H program.

A copy of the application will be emailed to the staff person applying for the grant and a copy will be emailed to the supervising 4-H educator for approval.

NOTE: Before applying for a 4-H Participation Fee Scholarship Grant, it is expected that efforts have been made locally to raise funds at the club and county level to cover 4-H participation fees for individuals and families. You will be asked to document the efforts that have been made and the level of success that was achieved.

  • A confirmation email with a copy of this application will be sent to this address.
  • 1. Amount of Scholarship Grant Needed?

    Applicants can apply for amounts from $100 (minimum 5 individual youth) to $500 (maximum 25 individual youth). Scholarships can be for individuals or families with four or more children enrolled in the program.
  • ($20 per individual or $60 per family with three or more children in the program)
  • This represents the number of $20 scholarships for individual 4-H members enrolled in the county 4-H program during this program year (the program year about to end).
  • This represents the number of $60 scholarships requested for families who have three or more children enrolled in the county 4-H program during this program year (the program year about to end).
  • 2. Describe your efforts at the club and county level to generate funds for the individuals/families you are requesting scholarship funds for:

  • 3. Describe the type of 4-H opportunities youth receiving participation fee scholarship will be involved in?

  • (e.g. list 4-H club or group name, content focus of youths’ participation, etc.)
  • A copy of this grant application will be emailed to the supervising 4-H staff person for approval.

Note: All Participation Fee Scholarship grant applications are due by June 1.
